Sales Tax
We are required by law to add sales tax to goods shipping into the following states:Calif.- 6.25% .

Payment Options
We accept Visa, Master Card or American Express for all types of orders (internet, phone, fax, mail). Open account terms may be extended to business or government agencies pending credit application and approval. If you have an existing open account, all that is required when ordering via the internet is a Purchase Order number (may fill in with verbal). Upon submission of your order, UMD will verify your account information prior to processing the order. International orders shall be placed by approved credit in the favor of UMD.

Damaged Goods
In the event you receive a damaged or short shipment, you must immediately file a claim with the carrier. Under ICC regulations this claim is the consignee's (your) responsibility. Please contact our customer service department 1-888-227-5948 or karend@unimeddesign.com for assistance.

Returns
When returning any part of your order, please contact our customer service department FIRST at karend@unimeddesign.com (or 888-227-5948) and include your UMD order# and the specifics of your situation. Return Merchandise Authorization number (RMA#) must be issued before return and must be on all packages and/or packing list . Carefully repackage the merchandise using the original packing material if possible, to prevent damage in transit. All items that are returned should be in the same condition as when delivered. Packages can be sent to UNIVERSAL MEDICAL DESIGN, LLC 22615 Sliverlode Road Palo Cedro, CA 96073. Please insure it for full value and keep all your postal and insurance receipts. Cancellation or alteration of an order or return of any product by BUYER may not be made without advance written consent by UMD and at UMD’s option, shall be subject to a cancellation, alteration or return charge of 25% of gross amount (on custom orders only) of invoice. Credit memo will issue for any balance due and can apply to other outstanding invoices. All credit card sales are subject to all terms and conditions as stated above.

IMPORTANT
Custom-made or specialty items may not be returned unless error was clearly ours. We will do our best to allow you to receive maximum value if you decide to have these custom-made items re-worked, repaired or replaced.

If you are purchasing products in quantities or are in need of a special or new product which is not found in our catalog, we are happy to provide you with a special price quote. Please contact us for the necessary forms.
